Is After-Care Planning Right For Your Needs?

  • What It Is

After-Care Planning helps your child transition smoothly from treatment back into everyday life — and stay on track once they're home. This includes identifying the right resources for your child's return (psychiatrists, therapists, IOP/PHP programs, tutors, mentors, life coaches, schools) and building a clear, structured plan so progress made in treatment doesn't get lost in the transition.

  • Who It's For

Families whose child is coming home from a short-term or long-term therapeutic stay — wilderness, residential treatment, or therapeutic boarding — and needs a thoughtful plan and real support system as they adjust to added stressors outside of treatment.

  • What To Expect

I start with an honest assessment of what's really going on, get a thorough understanding of your child's clinical recommendations, and identify the resources and support your family needs. From there, I help coordinate the right combination of providers — psychiatrists, therapists, IOP/PHP programs, tutors, mentors, life coaches, schools — so your child has real structure as they return to daily life. I stay involved with ongoing check-ins along the way.

  • Why It's Effective

It's important to have a well-thought-out plan, created by someone with a wealth of resources at her disposal. A strong after-care plan protects the progress your child made in treatment, reduces the risk of relapse or regression, and keeps your whole support team — family and providers alike — aligned.
